Indiana Annual Withholding Reconciliation Form WH-3

Effective January 1, 2015, all employers with Indiana withholding requirements must file the 2015 WH-3 (Annual Withholding Reconciliation Form) along with the state copy of the W-2s by February 1, 2016. The instructions to this form reflect the old due date which was on or before the last day in February.

This Form also applies to Indiana withholdings from nonresident contractors, nonresident partners, nonresident shareholders and nonresident beneficiaries. The state copy of the applicable forms (W-2G, WH-18 or 1099) should be attached to the WH-3. Forms can be found at:https://www.in.gov/dor/4100.htm
